Your discharge from Ungweru Specialised Medical Centre (USMC) is a positive milestone on your path to recovery. To ensure a smooth and confident transition home, we have prepared this guide. Our care for you doesn’t stop when you walk out our doors. We are committed to empowering you with the knowledge you need for a safe and successful healing process.

The Discharge Process: What to Expect

On the day of your discharge, our team will coordinate to ensure everything is in order before you leave.

  1. Final Specialist Review: Your doctor will conduct a final assessment to confirm you are medically ready to be discharged.

  2. Discharge Instructions: A nurse will review your personalized discharge plan with you and a family member or caregiver. This is the perfect time to ask any final questions.

  3. Finalising Administration: Our reception staff will help you finalise any billing and provide you with your medical documents.

Your Recovery at Home: A Personalised Action Plan

Following your discharge instructions carefully is the most important thing you can do for your recovery. Your take-home folder will include specific details on the following:

  • ✅ Your Medications:
You will receive prescriptions for any necessary medications. Please ensure you understand the dosage, timing, and purpose of each one. It is crucial to complete the full course of any prescribed antibiotics.

  • ✅ Incision & Wound Care:
You will be given specific instructions on how to care for any surgical incisions. This typically includes keeping the area clean and dry, and knowing when to change dressings.

  • ✅ Activity & Rest:
Your body needs energy to heal. Your guide will outline any restrictions on activities such as lifting, exercising, and driving. Getting enough rest is just as important as gentle movement.

  • ✅ Diet & Nutrition:
You may have specific dietary instructions. Generally, it’s best to start with light, simple meals and drink plenty of water to stay hydrated.

Your Follow-Up Appointment

Continuing your care with a follow-up visit is a critical part of your treatment plan.

  • Scheduling Your Visit: Our team will either schedule your follow-up appointment before you leave or will provide clear instructions on when and how to book it.

  • Purpose: This appointment allows your specialist to check on your healing progress, review any test results, and answer any new questions you may have.
